About This Policy
This Privacy Policy explains how Northeast Florida Wound Cure and Hyperbaric Medicine, PLLC, doing business as Gable Wound Care & Hyperbaric Medicine (“Gable,” “we,” “us,” or “our”), handles information collected through gablemed.com (the “Site”).
This Policy is not our Notice of Privacy Practices. Protected health information that we create or receive in the course of providing health care to you is governed by the Health Insurance Portability and Accountability Act (“HIPAA”) and by our Notice of Privacy Practices. If any provision of this Policy conflicts with our Notice of Privacy Practices with respect to protected health information, the Notice of Privacy Practices controls.
Information We Collect
Information you provide. The Site does not collect information through online forms. If you contact us using the telephone number, fax number, or email address listed on the Site, you provide the information you choose to include in that communication. Email is not a secure method of communication. We ask that you not send medical details, photographs, insurance identification numbers, Social Security numbers, or financial account information by email. Please call our office to discuss your condition.
Information collected automatically. Our Site and our service providers may automatically collect technical information including IP address, browser type and version, operating system, device type, referring page, pages viewed, and the dates and times of visits. We use this information to operate, secure, troubleshoot, and improve the Site.
Cookies and Similar Technologies
The Site uses cookies for essential functionality and for aggregate analytics that help us understand how visitors use the Site. You may disable cookies through your browser settings; portions of the Site may not function properly if you do.

Message frequency varies. Reply STOP to opt out at any time or HELP for assistance. Carriers are not liable for delayed or undelivered messages.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third party, except with service providers that deliver messages on our behalf and are bound to use that information only for that purpose.
Service Providers
We use third-party vendors to host the Site and to provide aggregate analytics. These vendors do not receive protected health information through the Site. Any vendor that creates, receives, maintains, or transmits protected health information on our behalf operates under a written business associate agreement with us as required by HIPAA.
Security
We maintain administrative, technical, and physical safeguards designed to protect information in our possession. However, no method of transmission over the internet or method of electronic storage is completely secure, and we cannot guarantee absolute security.
Do not use unencrypted email to transmit urgent or emergency information. Email is not monitored continuously. In an emergency, call 911.

The Site may contain links to third-party websites, including our patient portal. We are not responsible for the privacy practices or content of sites we do not control. Review the privacy policy of any site you visit.
Children’s Privacy
The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site.
